Medication Treatments

  • Oral contraceptives: Combined pills containing estrogen and progesterone may be prescribed to induce regular menstruation and artificially reduce certain symptoms. However, it is important to note that these medications do not cure PCOS and may have undesirable side effects, such as nausea, headaches, irregular bleeding, loss of libido, or weight gain.

  • Anti-androgens: Hyperandrogenism is one of the most difficult issues to manage in the context of PCOS. Spironolactone is a medication that can be used in the treatment of polycystic ovary syndrome (PCOS), especially to alleviate symptoms related to excess androgens (male hormones) in women with this disorder. Spironolactone is an aldosterone diuretic that also has anti-androgenic properties. It works by blocking androgen receptors, thereby reducing the effects of male hormones on the body.   • Androcur: Androcur is a medication containing the active ingredient cyproterone, which is an antiandrogen. In the context of hirsutism, Androcur works by reducing the production of androgens, the male hormones present in small amounts in women. Androgens can contribute to excessive hair growth in areas such as the face, chest, abdomen, and back. By inhibiting the effects of androgens, Androcur can help reduce unwanted hair growth. These oral medications are commonly prescribed to induce ovulation, with Letrozole originally intended for breast cancer treatment but now utilized off-label to stimulate ovulation. Injectable gonadotropins and in vitro fertilization (IVF) are also viable options.

     

Surgical Treatments

In some cases, when medication treatments are not effective or appropriate, surgical options may be considered.

  • Ovarian drilling is a surgical procedure that involves small perforations of the ovaries, usually performed by laparoscopy. This technique aims to reduce the production of male hormones in the ovaries, which can help restore hormonal balance and promote regular menstrual cycles. However, ovarian drilling is generally considered a second-line option and is not recommended as a first-line treatment. Ovarian drilling also has a "transient" effect; after 6 months to 1 year, the benefits of drilling diminish because the cause of PCOS is not treated.

In cases where women with PCOS have difficulty conceiving naturally, in vitro fertilization (IVF) may be recommended. IVF is an assisted reproductive technique where ovulation is stimulated, eggs are retrieved and fertilized in the laboratory, and then the resulting embryos are transferred to the uterus.

A natural approach can also be beneficial in managing PCOS and help alleviate some symptoms. It may include lifestyle changes such as a balanced diet, regular physical exercise, stress management, weight loss, and maintaining a healthy weight.
